arrive at North Medical Center
30 minutes prior to your first
scheduled appointment. You
will be asked to provide information
for insurance and financial
purposes. Please bring identification,
insurance cards and be prepared
to pay your co-pay.
After your initial visit,
you will need to check-in
at the designated reception
area to verify your personal
information such as name,
address, phone number, marital
status, employment, and insurance
carrier. Please be prepared
to pay your visit co-pay.
