Prescription Refills, Nurse Call Back, and Referrals
Need a prescription refill? To request a prescription refill call 315.452.2828 or send us an email. NMPC providers will address your request within 48-72 hours of receiving your request.

Have a question for your doctor or nurse? It is NMPC policy that all calls are returned within 24 hrs. You can contact your provider team by calling 315.452.2828 or send us an email at messagecenter@northmedicalpc.com.

If you would like a copy of your medical record, please click here to download a Medical Release Form. Submit the completed release form to your doctor’s office or the facility that holds your records. We cannot provide medical records based on an email or online form.
