You can search for the kind of doctor you need by name and/or location. You’ll find background on each one of our doctors, including addresses, phone numbers and more.

Request an appointment

To make an appointment, please call 315-452-2828 or 315-637-7878 and a patient services will be happy to assist you.

Prescription Refills, Nurse Call Back, and Referrals
Need a prescription refill? North Medical requires 72 hour notice for refill requests. Please be aware that many our providers require patients schedule an appointment to refill a prescription. Federal Law prohibits us from issuing a prescription for controlled substances by phone.

To request a prescription refill call 315-452-2828 or send us an email.

Need a referral to another provider? It is North Medical policy that all referral requests are addressed by the clinical team. You can contact your provider team by calling 315.452.2828 or send us an email at messagecenter@northmedicalpc.com.

If you would like a copy of your medical record, please click here to download a Medical Release Form. Submit the completed release form to your doctor’s office or the facility that holds your records. We cannot provide medical records based on an email or online form.
